A scheduled appointment is needed for all diagnostic testing performed at Virginia IVF & Andrology Center.

To schedule an appointment, please call the Andrology Laboratory at (804) 323-9980 between 8:00 AM and 4:00 PM Monday-Friday

Please have your physician's written order and your insurance card on hand when calling to schedule your appointment. Your written order from your physician or nurse practitioner must include the following:

Specific Test Ordered (checked or written)
Male Name
Diagnosis Code
Physician Signature

To save time filling out paperwork at our office, you may wish to printout and complete the Patient Information Form to bring with you.

Please remember to bring your written order, insurance card, photo ID and your payment with you on the day of your appointment. We are unable to perform any test without a written order.
